I fratelli Mantle sono gemelli identici (ambedue interpretati da Jeremy Irons), brillanti ginecologi. Claire Niveau (Geneviève Bujold) è un’attrice che vorrebbe un figlio. Il responso alla clinica dei Mantle è però negativo: a causa di un problema congenito, non potrà averne. Elliott Mantle è interessato a Claire e va a letto con lei. Poi invita il gemello Beverly a fare altrettanto, senza informarla dello scambio: è una cosa che i Mantle fanno spesso. Beverly però stavolta si innamora. Claire scopre il giochetto e si arrabbia. Beverly ne è amareggiato e si dà all’alcol e alla droga. Tempo dopo Beverly reincontra Claire e i due si mettono insieme, ma ormai la droga sta prendendo il sopravvento e la salute mentale di Beverly peggiora: ossessionato dall’idea delle donne mutanti, commissiona la realizzazione di bizzarri strumenti ostetrici.
